AFFIDAVIT OF PROOF OF SERVICE

I, OLIVER PELENO of legal age, single, Filipino, with postal address at

2711 Francisco St. Gagalangin Tonda Manila City, after having been sworn to in
accordance with law, depose and say:

1. That I am employed with JABEZ LIAN TRANSPORT AND TRAVEL

SERVICES INC. as a messenger;

2. That as a messenger I am authorized by said company to serve copies

of demand letters, notices and other documents to the clients and
employees of the above mentioned company

3. That on June 4, 2015, I have personally served to Melissa Salonga a

letter Notice to Explain dated June 4, 2015 at her residence at 2328
Yangco St. Gagalangin Tondo Manila.

4. That Melissa Salonga refused to receive the said Notice To Explain

Letter. Nevertheless, a copy of the same was left at her premises;

5. I am executing this Affidavit to attest to the truthfulness of the foregoing

facts.
