DIVISION MEMORANDUM

2. The format of the training proposal is based on DepED Order No. 66, s, 2010,
re, Policies and Guidelines on Planning and Administration /Management of the
Human Resource Training and Development Program. It includes the following
data:
a. Program Title
b. Background and Rationale ( description/discussion of the conditions that
justify the need for training supported with qualitative and quantitative data);
c. Description of the Project/ Program;
d. Goals and Objectives (deliverables);
e. Strategies and Subprojects;
f. Targets by training program or sub-project, project locale and level of
participants;
g. Implementation Mechanics (the Training Design Matrix);
h. Fund Source/ Project Partners / Donors ;
i. Work and Financial Plan;
j. Monitoring and Evaluation Scheme
5. The Training Design Matrix which is in tabular form is the data to be placed
under the Implementation Mechanics , Letter g of the training proposal. A
sample matrix is attached in this memorandum,
6. The format for the Training Completion Report (TCR) should be able to provide
very relevant information to the head of office or head of the agency. It shall
follow the format below.

II. SUMMARY OF TRAINING EVALUATION
7. All teachers are advised to revisit their IPPD's last year and should do
something about it. The school heads should also revisit their SPPD's last
year and submitan accomplishment report or a Re entry plan for the
realization ofthe professional plan of development intheirschool.
8. The use ofthe training passbooks is also emphasized. INSET Coordinators
are advised to consolidate at theend ofevery semester thetrainings attended
bythe teachers and staffin their school. Training needs should also be
consolidated and be utilized as basis in conducting trainings orin sending a
teacher/ staff to seminars. They should start doing a data base ofthese
information.
