Matthew Fedowitz. All affected site workers will review, be allowed to incorporate Site Specific as per ES&H Plan
appropriate comments, and document that they understand and will abide by the JHA
during a training session prior to initiating work on the subject tasks. Initial PPE: Level D. It is unlikely that these tasks will require other than level D
protection. However, the PPE may be upgraded to Modified Level D, Level C, etc. in
accordance with professional judgement and Paragraph 6.0 of the ES&H Plan.
Tools and Equipment: Tools employed may include, but are not limited to, walk-behind
asphalt saw, extension cords, 3” metal pipe, pipe thread-lock and cleaner, and hand Approved:
operated digging implements.

Description of Work:
Remedial Action for cutting a three-inch wide and three inch deep trench from curb to the security trailer in order to establish permanent power. The cord will then be slid
through 3” wide metal plumbing pipe, which will then be covered with asphalt patch so traffic can continue as normal without the installation of a temporary speed bump.

Slips, Trips, Falls • Clear walkways, work areas of equipment, tools, carts and debris.
Using Walk-
• Mark, identify, or barricade other obstructions.
Behind
• Keep cords out of areas where the saw may cut them.
Asphalt (wet)
• Since water will be used to cool the saw blade, workers will use caution on wet surfaces.

High Ambient
Temperature • Employees shall be protected from exposure to heat and cold by complying with Paragraphs 5.6 and 5.7 of the
ES&H Plan.
• Provide fluids to prevent worker dehydration.
Handling Heavy • Back strain or injury prevention will be discussed routinely as a safety topic.
Objects • The hands and the object should be free of dirt or grease that could prevent a firm grip.
• Due to the weight of the walk-behind saw, crew will not attempt to lift it for any reason.
Sharp Objects • Keep guards in place during use and do not operate saw if guards are not in working order. Workers will consult
operations manual before commencement of work.
• Ensure the saw is turned off prior to changing blades (if necessary).
• Don’t use a dull blade; replace or have sharpened prior to use.
• Be alert for sharp points and edges. Wear cut resistant work gloves when sharp edges or objects may cause the
possibility of lacerations or other injury.
• At no time will any crew member stand in front of the saw while in operation
High Noise Levels • All crewmembers within a 100-foot radius of cutting operations are required to use hearing protection.
Struck By/ Against • Wear reflective warning vests when exposed to vehicular traffic.
Heavy Equipment • Make eye contact with operators before approaching equipment.
• Do not attempt verbal communication in high noise backgrounds.
Contact with moving • Work area will be barricaded/demarcated. No traffic will be allowed to move past the cutting area until crew is
equipment / vehicles complete with cutting and asphalt patch has been applied.
• Barriers shall be used to protect workers from vehicular traffic.
• Identify emergency response providers/hospitals.
Emergency Situations • Follow the direction of your DOS representative during an emergency situation within the facility.
Pinch Points • Review equipment adjustment procedures, identify pinch points.
• Isolate/block pinch points to limit motion when inserting pins, fasteners, closing tackles.
• Keep hands, fingers, and feet clear of moving/suspended materials and equipment.
• Stay alert at all times.

Extension cords • Extension cords that are unsafe due to faulty plugs, damaged cords, or other reasons, shall be tagged (do not use)
and removed from service.
• Cords shall be protected from damage from sharp edges, projections, pinch points (doorways), and vehicular
traffic.
• Cords shall be suspended with a nonconductive support (rope, plastic ties, etc.) when appropriate.
• Cords shall be designed for hard duty.
• Cords shall be inspected daily.
