MEMORANDUM OF AGREEMENT

This memorandum of agreement is made and entered into by and between:

(Barangay/GPTA/and other organizations), is a duly organized and existing (Local

Government Unit/Parent Association/Pupils Organization), with principal addresses at the
______________________ represented herein by (President of ____________________ or
any representative of the organization) and hereinafter referred to as the FIRST PARTY.

The (name of school), District of Bocaue, Division of Bulacan, represented by the

School’s Principal, (name of Principal), (SBM Coordinator, and other if you want to add in your
respective representative) with principal addresses at the ____(address)________ and
hereinafter referred to as the SECOND PARTY.

WITNESSETH:

WHEREAS, the (Barangay /GPTA/and other organizations) and the (School) are concerned
about the welfare of the pupils and the school in achieving sustainable quality basic
education;

WHEREAS, the (Barangay /GPTA/and other organizations) and the (School) are committed to
improve the quality of basic education of the pupils, through the joint effort of the community
and the school.

WHEREAS, both parties believe that the major key reaching the goal of quality education is to
develop and support the programs that will provide the pupils, teachers and school the
opportunity to be equipped with full potentials that they may be productive citizens in the
future.

NOW THEREFORE, premises considered, the PARTIES agree to the following:

The (Barangay /GPTA/and other organizations) shall

1. coordinate with the (School) on the projects to be implemented

2. conduct regular visits/attend meetings
3. provide activities for the improvement of the program/project
4. Serve as the community support system of the school

The (School)

1. Conduct orientation/briefing of the projects to be implemented

2. Allow the (representatives of the barangay) to join in certain activity/activities in line
with the program/projects related to SBM.
 3. Monitor and evaluate the conduct of the said program joined by the (Barangay
/GPTA/and other organizations).
4. Submit consolidated evaluation report on the said projects/programs.

This AGREEMENT shall take place effect immediately upon signing by all parties concerned
and shall remain in force unless terminated by mutual consent.
