Type of Injury Brief description of accident

Sh. Ansar Khan was grinding on impeller blade which is kept in vertical position on
uneven surface. Due to uneven surface and not having proper support the impeller
Fracture was fallen on his both legs causing fracture in his left Thigh Leg.

On dated 12.05.17 at 09:30 AM cleaning job was allocated to two labors near belt 5B

Type of Injury Brief description of accident

While handling of gas cylinder with trolley due to hose clamp minor cut injury in his
Cut hand palm.

While working in APH dust fallen, he run away from the dust causing tube bundle hit
Blunt his back.

Unknown bite While cutting of plantain may reptile bite / Unknown bite to his foot.

On Dated 18-08-2017 at 06:30 PM he started working at Unit-1 Fly Ash conveying

Line No.1B switch valve towards Fly Ash Silo and during that period the switch valve
of the branch pipe named as Recirculation Line got pass. And suddenly the Fly Ash
got sprayed around due to back pressure and edge found cut of the switch valve.
Dust fall in And some amount of Fly Ash sprayed over the face of Mr. Kamarudin and his eyes
eyes got contact with fly ash. Immediately his eyes cleaned by drinking water & First Aid
Treatment given at our Mech. Store. After that he sent to SCL dispensary for further
treatment. Considering as a first aid injury by Doctor, treatment provided. Now he is
well condition.
