AFFIDAVIT OF GUARDIANSHIP

I, ROSELYN P LAPIZ, of legal age, Filipino citizen and a resident of

Zone 7, Target Range, Camp Edilberto Evangelista, Patag, Cagayan de Oro
City, after having been duly sworn to in accordance with law, do hereby
depose and say:

That, I am the common-law wife of the late Sgt Jonell P

Montederamos 827506 PA, who died on 08 October 2019 at DOA-
Polymedic Medical Plaza, Cagayan de Oro City;

That, out of our union, we begot three children, namely: Job II Lapiz
Montederamos, who was born on 25 July 2013; Jonell Gavrell Lapiz
Montederamos, who was born on 01 May 2015; and Isiah Jonnel Lapiz
Montederamos, who was born 15 April 2017, and are among the
beneficiaries of the late Sgt Jonell P Montederamos 827506 PA;

That, I am the guardian of the above-mentioned minor children who

are under my care and custody;

That, I am executing this affidavit to attest to the truth of the

foregoing and for whatever purposes it may serve.
