June 22, 2018

The meeting started at 8:00 o’clock in the morning with a prayer by Lorena O. Macahine (Parent).
The class adviser presided the meeting tackling the first agenda which is the election of the new set
of hpta officers for the school year 2018-2019. The election has been successful.

In the second agenda, the class adviser emphasizes the importance of PSA Birth certificate as a
school priority record in the validity of pupil’s information. The class adviser also gave the schedule
of submission of PSA Birth certificate for those who did not submitted yet. The parents agreed to
this memorandum.

In the third agenda, the class adviser reminded the parents about the required paraphernalia’s of
their child in the opening of classes. The parents also had agreed to provide their children all the
required school materials.

All other concerns were discussed both the adviser and the parents.

The meeting ended at 10:00am.

The meeting started at 9:00 o’clock in the morning with a prayer led by Armonia Ongkoy (Parent).
The class adviser reviewed the first meeting agenda on June 22, 2018, after which is the reminders
on PSA Birth certificate to those who did not submitted yet. The adviser proposed the exact date for
submission.

In the second agenda, the class adviser reminded all the parents about the pupil’s attendance in the
school. The class adviser gave advised to the parents to monitor their child whenever they are in
school. “It’s the responsibility of the parents to supervised and monitor your child” according to sir
Jim.

In the third agenda, the parents have agreed to attend tagbo for the garden. Which will be on August
18, 2018. Bring bolo, patong and kahoy and sani

In the 4th agenda, fiesta celebration is approaching, the school initiated the activities for the DepEd
Night and it is the Search for the parents. And the grade VI class chose Jinalyn Maduay to join.

All other concerns were discussed both the adviser and the parents.

The meeting started at 2:00 o’clock in the afternoon with a prayer through audio visual presentation.
The class adviser presided the meeting in reviewing first the last meeting which was held on August
16, 2018. Still in the first agenda, other parents failed to submit their child’s PSA Birth Certificate.
The class adviser gives another date for submission.

In the second agenda, the class adviser reminded the parents about their child attendance. Monitor
always “Lorena Macahine said”

In the third agenda, the adviser also emphasizes the performance of the pupils in the school. The
parents agreed to limit their child in watching tv during night time for them to be ready for the next
day. And the next agenda, the Christmas party for the children. The parents agreed to have baon
for the Christmas party with a gift of over Php100.oo worth.

All other concerns were discussed both the adviser and the parents.

The meeting ended at 4:00pm.

The meeting started at 8:00 o’clock in the morning with a prayer through audio presentation. The
class adviser presided the meeting tackling the first agenda which is the number 1 agenda of all
quarters. The adviser gives ultimatum for those who did not accomplish it.

In the second agenda, the class adviser reminded all the parents about the pupil’s attendance in the
school. The class adviser gave advised to the parents to monitor their child whenever they are in
school.

In the third agenda, the class adviser reminded the parents about the schedule of checking, that
during this time, all records should be checked by the principal. And the last agenda, the class
adviser announced the schedule of graduation and the agreement regarding this matter.

All other concerns were discussed both the adviser and the parents.
