Absorbed Toxins
By precipitating or adsorbing the fluid toxin with alum, aluminum
hydroxide, or aluminium phosphate, an adsorbed toxin is produced, which
when administered , results in a slower release of the antigen from the site
of injection and subsequent production of higher and prolonged antibody
titers.
However, adsorbed toxoids are more prone to produce local reactions
at the site of injection than are fluid toxoids.
To avoid this , adsorbed toxoids should be administered by the deep IM
injection , whereas the fluid toxoid may be administered subcutaneously.
Use:
Both fluid and adsorbed toxoids are used to produce active immunity
against diphtheria and tetanus.
They are used alone and in combination.
In young children, diphtheria and tetanus toxoids combined with the
pertussis vaccine is often used , and the combination is commonly known
as triple antigen or DTP.
Antitoxins are prepared form the blood of animals , usually horses that
have been immunized by the repeated injections of specific bacterial
exotoxins.
ANTITOXIN UNITS
TETANUS ANTITOXIN
BOTULISM ANTITOXIN
This antitoxin contains not more than 20% of solids and should be
stored at a temperature between 2 and 8C.
The expiration date is not later than 5 years after the date of issue.
A multivalent antitoxin
Used to treat all cases of toxemia caused by the type of botulin’s
bacteria used in its preparation.
A multivalent antitoxin is advantageous because the prescribing
physician is not required to wait for a determination of the type of
causative organism.
VENOMES
Poisonous excretions produced by animals: they can be compared
with toxic waste products (exotoxins) of bacteria.
The detrimental effects developed in humans and animals following
the bites of poisonous snakes (rattlesnake, copperhead, cobra, others)
have been known for many years.
About 10,000 people are bitten by poisonous snakes every year in
United States.
Poisonous snake bite often cause severe pain and can lead to tissue
necrosis, amputation and death.
The venom of rattlesnake is a complex mixture, chiefly proteins,
many of which have enzymatic activity and a nonenzymtic
neurotoxic fraction.
Similarly the venoms of the tarantula, scorpion, black widow spider,
honeybee, wasp and other arthropods produce various deleterious
effects of depending on the amount, time of year, and other
conditions.
BUFOTOXINS
ANTISERUMS
At the present time, rabies immune globulins are preferred for this
combined therapy: however if it is not available antirabies serum can
be used after testing the sensitivity.
USE AND DOSE:
Provides the patient with immediate protection against rabies.
The biologic should bear an expiration date not more than 2 years
after the date of issue.
It is available in containers of 1000 units.
The usual dose is IM, not less than 1000 units per 40 pounds of body
weight.
Part of this serum dose should be infiltrated into the tissue around
the wound whenever feasible.
IMMUNE GLOBULINS
IMMUNE GLOBULIN
Sterile solution of the globulins derived from the body of healthy adult
human donors who have been previously hyper immunized with mumps
virus vaccine.
This immune globulin is used for prevention and treatment of mumps in
adults and childrens.
Can be used in combine with rabies vaccine as the best post exposure
prophylaxis.
Usual dose is a single administration of 0.133 ml per kg of the body
weight at the time of the first vaccine dose.
Up to half of the dose should be use to infiltrate the wound and rest
administered intramuscularly.
