Re: Ms Amber Watson, DOB: 25/03/1999

I am writing to refer Ms Watson , who was tested positive for chlamydia trachomatis in our facility.
Therefore, your further care and management is higly requested.

On initial visit, Ms Watson came in for a request of Pap test. Aside from that, she was also worried from
her weight gain of 10 kg in 1 year. Furthermore, she asked about different types of family planning
methods. In general, Ms Watson has no known allergies neither medical nor surgical history. She was a
smoker and an occasional drinker also.

With regards to her sexual and reproductive history, she has regular 28-day menstrual cycle. Moreover,
her LMP was recorded 7 days back and no history of STI nor pregnancies. Although she claims to have 1
partner currently, it was not sure that it was monogamous. Please note that she is on oral contraceptive Comment [Office1]: she

On her follow up consultation, Pap test and PCR test reveals positive for chlamydia trachomatis, hence, Comment [Office3]: revealed
safer sexual practices which includes barrier protection were discussed. Consequently, azithromycin 1 gm
single dose was commenced and instructed to refrain from intercourse for 7 days of treatment. In
addition, a referral to Outline and women’s health nutritionist were recommended. Comment [Office4]: Quitline

In view of the above, kindly facilitate additional laboratory test for sexually transmitted diseases and blood
borne viruses. Finally, it would be appreciated if you can discuss further methods of contraception.
