AFFIDAVIT

I, ____________________ of legal age, married and a resident of

_____________________, Sorsogon City, after being sworn in accordance with
law, do hereby depose and state:

1. That I am the Area Supervisor of Palawan Pawnshop and Palawan

Express Pera Padala with business address at Magsaysay St.
_______________, Sorsogon City;
2. That Palawan Pawnshop and Palawan Express Pera Padala is leasing a
Commercial Stall/Space in a commercial building at Magsaysay St.
___________, Sorsogon City, owned Rosalinda Paulos;
3. I likewise and make manifest that I am applying for a registration for
Mayor’s Permit for the said business;
4. I further declare that the aforementioned permit was not duly issued
sans the compliance of the requirements for a building occupancy
permit of the commercial building;
5. That I execute this oath to undertake that within two (2) months the
execution of this oath I will submit the necessary requeirements for the
issuance of the Business Permit, Building permit and for Application for
electrification of the said Commercial Stall/Space;
6. That I am executing this affidavit as a requirement for issuance of
Business Permit, Building Permit and Application for Electrification in
the said commercial stall/space.
