FINAL CLEARANCE GUIDELINES

ATTENTION GRADUATING STUDENTS!


You only have 3 days clearance. Follow the scheduled date and time for your respective section
or you will lose your turn. Clearance will be on a first come first served basis. Present the clear-
ance forms/checklist that was given to you during 3rd year (to avoid redundancy of checking) and
the clearance checklist for second semester.

You will not be allowed to attend the baccalaureate mass and solemn investiture if you are
not cleared thus make sure all deficiencies are cleared with all your CIs, Lecturers & Regis-
trar’s requirements. You may approach your clearance officers for partial clearance and/or
advising/clarifications.

PREPARE THE FOLLOWING:

3. OR/ DR CLEARANCE FORM (duly signed by the OR/DR clearance officers/CIs)

4. SCRUB FOLDERS ( OR / DR )/COMPETENCY CHECKLIST ( for those not yet


signed by OR/DR clearance officers)
➢ Duly signed by respective clinical instructors
➢ Data input should be exactly the same as you have entered in your greenbook.

5. PRC WHITE FORM & RLE FORM (for those who have not yet submitted)
➢ Data must be written in BLACK ballpen not in sign pen.
➢ Duly signed by designated OR / DR CIs.
➢ NOTE: FABELLA PRC FORMS – make sure name of staff nurse is indicated.

6. EXAMINATION PERMIT FOR Prelims & FINALS


➢ Make sure that you keep your 2nd semester permit for Prelims & Finals. If you lost it,
have a copy from Accounting Division right away to prevent delays.
➢ Staple it on top of the receipts.

7. RECEIPTS
➢ Staple all the receipts in-order as stated for easy checking:
1. Graduation Fee/Commencement Fee – treasurer to collect to be forwarded to _____
2. Alumni (USTNAAI) - c/o Ms. Rolisa Almario (Alumni Treasurer)
3. Review Fee - c/o Accounting Division
4. Pins - c/o Ms. Josie
5. Toga - c/o Genecar Pe Benito * make sure head piece is well fitted
1. Gala receipt - ( for female ) - by Aling Ising
2. CAPS – c/o Josie
3. Enrichment Class Fee
* Payment of RLE make-up; OR/DR completion

8. From the Office of the Alumni Relations


> Alumni Card (UST)
> Information Sheet (short bond paper)

9. SHOES & UNIFORM


Male: White Duty Shoes
1. Must have the prescribed duty haircut
2. Moustache and beard MUST be shaved
NOTE: Long hair will be trimmed during graduation
Moustache and beard will be shaved during graduation

Female: Plain white, close shoes, maximum of 1- 2 inches high.


NOTE: (during graduation)
3. HAIR on a BUN
4. NO Wedge, sling back shoes, peep toe shoes, stilettos, non-pointy
5. NO Hair color of any shade
6. NO Colored Nail Polish
7. NO dangling earrings
8. Use SHEER WHITE STOCKINGS

 YOU WILL BE CONFINED IN A HOLDING AREA AND CHECKED PRIOR


TO MARCHING TO BE SURE THAT YOU ARE PROPERLY ATTIRED
AND KEMPT.

Bring borrowed gala for fitting. Make sure that it is clean and pressed.

10. OUTFIT DURING THE CLEARANCE DAY


➢ First day of clearance
MALE:
> Duty uniform plus white shoes to be worn during graduation day
FEMALE:
> Base uniform

➢ Succeeding days after uniform for graduation has been checked :


 College shirts/UST shirt, long jeans (Type B policy applies)
 Close shoes/rubber shoes
 College ID
 Well-groomed hair
 No nail coloured nail polish
 No slippers/sandals, No shorts
* Other than the prescribed attire, you will not be entertained
(STRICT COMPLIANCE)

11. REVALIDATION
>Prepare for a revalidation of skills/concepts that was or was not done.

NOTE:
➢ Bring 2 expandable folders (green) for RLE Forms & OAR Information sheet
➢ PRC FORMS in exchange for CERTIFICATE of UNDERTAKING thus NO PRC
FORMS NO UNDERTAKING (which is a requirement for PRC application for board
exams).
