AFFIDAVIT OF DISCREPANCY

I, MARIAFE M. PLAZA, Filipino, of legal age, and married with postal address at San Roque Village,
Km. 9 Sasa Davao City, after having been sworn to in accordance with law, hereby depose and
state:

That in my Philippine Passport my name was written as Maria Fe M. Plaza, instead of Mariafe M.
Plaza;

That my true name is Mariafe S. Manatad as appearing in my Certificate of Birth and Certificate
of Marriage a copy of which are hereto attached as Annex “A”;

That in all pertinent documents including GSIS records and PRC records, I used my true and
correct name of Mariafe M. Plaza;

That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing facts and
to explain the discrepancy in my Passport as regards my true name;

That I assumed the name of MARIAFE M. PLAZA when I signed official documents in the exercise
my professional work in the government;

That the name Maria Fe M. Plaza as appearing in my Philippine Passport and Mariafe M. Plaza as
appearing in Certificate of Birth and all other pertinent documents refer to one and the same
person which is the affiant herein;

That I am executing this Affidavit to clear any discrepancy which may arise from official
documents bearing either name Maria Fe M. Plaza or Mariafe M. Plaza
