This is the incident report for the patient Rick Grimes created on December 3, 2016. On
December 2 at 18:45 hours, the patient Rick Grimes suffered from shortness of breath
which was witnessed by family members.
The patient has a family history of heart disease. His father died from a heart attack. Rick
suddenly displayed tachycardia and chest pain leading to shortness of breath.
After the incident, the patient had a blood pressure reading of 200/160 and a pulse rate
of 132. He was conscious and responsive.
The head nurse and doctor were notified of the incident. The patient was ordered to
receive IV Fluid of D5W 100 ml per hour and Nitroglycerine 0.5 mg SL initially and to be
repeated every five minutes for three doses.

TREATMENT ORDERED Broken wrist, the x-rays was reviewed to determine
if the fracture is in proper position. Broken wrist
amenable to cast. Bones were out of proper
position, light sedation was used to reset the
fracture. Pain killer was given.

This is the case of Betty Lee who was admitted to Saint Joseph Hospital on
December 28, 2016. One day earlier, the patient complained of wrist pain. It
was found out that she incurred a wrist fracture as a result of fall from her
hospital bed at 7:00 in the evening. The incident was witnessed by her
husband. Betty’s health history revealed that she had a previous fall from a
ladder 2 months ago. A doctor managed it by realigning the broken wrist.
The Physician and Nurse Supervisor were notified. The following treatment
and management were ordered. A follow up X-ray to determine if the
fracture has been corrected. It was done right away and revealed that the
bone was not in proper alignment. The Physician ordered resetting of the
fracture. Light sedation was administered during the procedure and followed
up with pain killers as ordered.
