Hospital Fall Incident Report

Mr. Gary Saunders, a patient at Mount Royal Hospital, was found on the bathroom floor by Nurse Tara at 1:05am. He had a 5cm laceration on his head and reported severe pain. His vital signs were recorded and he was conscious. The floor was wet and he had been on sedatives, which may have contributed to his fall. His wound was sutured by a physician at 1:35am and an x-ray was ordered to check for a skull fracture.

Time of Incident : 1:05am

Mr. Gary Saunders, a patient of Mount Royal Hospital was found on the floor
of his bathroom. Apparently, no witness was there for the incident. Nurse Tara
attended incident.
On the scene, the patient was lying on the floor. Head to assessment was
performed and an open injury of 5cm laceration was noticed over his bed. Patient
was conscious and oriented to the external environment. He reported severe pain in
his head. On observation, the floor was wet and medication chart shows the client
was on sedatives for his aggressive behaviour. These could be the contributing
factors to his fall. His vital signs were recorded as follows, Blood Pressure- 160/102
mmHg, Pulse: 110 Bpm, Respiration: 18 Bpm.. Patient was brought back to the bed
with assistance.
The physician was informed at 1:15AM. Simultaneously, acknowledged the
supervising Manager and the next of kin. The physician attended the patient and
sutured the wound at 1:35AM. Besides, the doctor instructed to keep ice pack over
the site of injury to relieve swelling. X ray was ordered to rule out the skull fracture.
The incident was recorded in the nurse’s chart.
