been duly sworn to in accordance with law hereby depose and state THAT:

1. I am an online seller;

2. As an ONLINE SELLER, I belong to a class of low income earners;

3. My yearly income is only THIRTY SIX THOUSAND (P36,000.00) which

is so meager and only enough to cover my daily needs;

4. I executed this affidavit to attest to the veracity of the above and in support
to my application for issuance of tax exemption.
