That I, KEVIN G. TANGONAN, single, of legal age, Filipino, and a resident of and
with postal address at Brgy. Maasin, Pura, Tarlac, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and says:
1. That I am applying for enlistment with the Philippine Army, AFP;
2. That I am still single/unmarried up to the present;
3. That as a requirement for enlishment, I hereby named, designated and
appointed my parents BENEDICT P. TANGONAN and SONIA G.
TANGONAN, both of legal ages, and residents of Maasin, Pura, Tarlac, as
my legal beneficiaries;
4. That I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal intent and purpose it may serve.
