REPORT OF INCIDENT

Dear Sir,

On 11th day of February year 2020, I was shock when I found out that a portion of
the sliding glass window was shattered and crack due to someone threw a bottle of beer
in the rear area. My room is the room 8 of the New Technology Building. Last time I was
there was the day before the incident happen and the said window was still no damage
at that moment.

In regards to this, I would like to report the said incident and if possible would like
to request for further investigation so that the same case would not happen again and the
suspect will be penalized for the damaged of property it has caused.

Hoping for an immediate action on this matter. Thank you and more power.
