Dear Sir,

I, , owner of stall, would like to request from your good office to add cooked food and snack under my
business permit. The issued business permit with a number of only includes retail selling in Sari-Sari
store.

I intend to add this product (cooked food and snacks) in my stall. I will be preparing and cooking the
food outside my stall but I’ll be using my stall to re-heat and retail them. There will be minimal usage of
the gas stove and will be solely used for re-heating.
