You should provide as much information as possible to obtain a replacement certificate:

1. Your full name;

2. Your date of birth;
3. Your City and Guilds enrolment number;
4. Date qualification was achieved;
5. Qualification title;
6. Centre name and centre number (see below).

If you did your qualification with Cool Concerns we can provide your enrolment number,
date achieved and the title of the qualification. Email your full name, date of birth and
qualification (e.g. F Gas, HC or NVQ) to info@coolconcerns.co.uk and we will email you
this information.

The current cost of a replacement certificate is £34 and you will need to provide
payment when making your request on the City and Guilds website.
