TO: All Development Management Officers (DMOs) and HRH Assigned in
the Different Municipalities/Cities in the province of Ilocos Sur
Yearly, the Department of Health (DOH) campaigns for a healthy and safe celebration of the
yuletide season. Despite the efforts with different partner agencies, a total of 340 fireworks-
related injuries was reported during the 2018 FWRI surveillance. The surveillance reports
coming from sentinel hospitals across the country provide data trends and valuable information
for timely and coordinated response from different DOH offices and agencies concerned with
the injury prevention and control program.
All DMOs and HRH assigned in the different municipalities are hereby directed to report daily
at 2PM all fireworks-related injuries from December 21, 2019 to January 6, 2020 in your areas
of assignment. HRH are also further advise to follow up cases, if there any, from hospitals in
your areas of assignment. To ensure that all information of the case is included in the report,
FWRI Surveillance Patient Information Sheet (Annex A) must be filled-out and submitted
within 24 hours from the time of the injury.
