1 OCI Application Form. In case of Minor both parent’s signatures and notary is Original
required on the last page of the OCI form.
Also minors below 5 year a thumb impression is required. Minors above the age

2 Photograph Original
 Please provide 2 most recent passport size color photos (not older than 6
months) of the applicant (hard copies).
 Photos to be 2 inches x 2 inches in size, in color, depicting front pose strictly
against white background only on photographic paper.
 Physical photo submitted to the CKGS and uploaded photo on the
government website has to be identical.

3 Proof of Address: - Copy
Proof of address must match with the present address as per application:
(any ONE of the below documents)
- State Issued ID OR
- Driving license OR
- Utility Bill - Water (should be recent, no more than 3 months old) OR
- Utility Bill - Gas (should be recent, no more than 3 months old) OR
- Utility Bill - Electricity (should be recent, no more than 3 months old) OR
- Valid Lease Agreement OR
- Mortgage OR
- Tax return OR
- For College/University Students living on campus in University
housing/dormitories, a letter from your institution on the institution letter
head explaining the exact address that you presently reside in will suffice.
NOTE: DO NOT CUT YOUR DRIVER’S LICENCE COPY DOWN TO SMALLER SIZE
Important: PO BOX number is NOT ACCEPTABLE as valid Proof of address.
4 Please provide copies of the following pages of your current valid US/ Foreign Copy
passport:-
- Information page that contains the picture and personal details of the
holder of the Passport.
- Endorsement/amendment pages.
Passport should be valid for at least 6 months from the date of submission of
application at CKGS.
5 Prepaid Envelope for assured dispatch of your documents Original
(Applicable only if you are using own prepaid for return shipping)
- Provide a prepaid envelope with Shipment Number and Service Provider
Name.
- FedEx / USPS prepaid self-addressed envelopes / shipping
labels will NOT be accepted by CKGS.
- Do not bring handwritten return envelopes as they will NOT be
accepted by CKGS
6 Mode of Payment:- Original
Payments can be made by Money order/ Cashiers/ Bankers check favoring “Cox
and Kings Global Services USA LLC"
Cash or Personal Checks will NOT be accepted.

7 Current US/ Foreign Passport (ONLY IF APPLYING FOR NEW OCI). DO NOT SEND Original + Copy- as
ORIGINAL PASSPORT IF APPLYING FOR OCI MISC. AND OCI IN LIEU stated in point no 4
OF PIO CARD above
8 Proof of Renunciation of Indian Citizenship (ONLY IF APPLYING FOR THE NEW Copy
OCI):
- If you ever held an Indian Passport, it is MANDATORY to provide proof of
Renunciation of Indian Citizenship along with your OCI application. One of the
following MUST be produced as proof:-
 Canceled Indian Passport with stamp stating the passport is canceled due
to obtaining US/Foreign citizenship OR
 Certificate of renunciation issued by the Embassy/Consulate of India OR
 Deemed surrender certificate issued by the Embassy/Consulate of India
Both - Cancelled/ Surrendered Passport along with the copy of the
Surrender/Renunciation Certificate would be needed for Indian passports
surrendered in or after 2010

If you do not have the proof of renunciation then please apply for renunciation
before submitting the OCI application - www.in.ckgs.us/renunciation
9 In case of a Minor, parents are requested to provide Copy
- a copy of parents’ passport
- a copy of parents' valid legal status in the US (for Non-US passport holders
only)
10 Proof of Indian Origin of Applicant (ONLY IF APPLYING FOR NEW OCI / Copy
RENUNCIATION)
- Copy of first three and last two pages of the last Indian Passport held.
- In case of non-availability of Indian passport, the decision to accept the
mentioned documents as proof of Indian Citizenship will be made by the
Embassy/Consular Officer and will be FINAL in such cases.

(if Indian Passport is not available, any One of the below documents):
- Birth Certificate OR
- Certificate of Indian origin from First Class Magistrate/District Magistrate OR
- Ancestral Nativity Certificate from Indian Official / Magistrate OR

If the applicant does NOT HAVE an Indian Passport and is applying for OCI on the
basis that his parents’ and/or grandparents’ were Indian Nationals, then the
applicant must enclose the following documents:-
- Copy of Proof of both parents’ or grandparents' current nationality and
previous nationality.
- Applicant’s Birth Certificate (in which their parents’ name is mentioned)
verified by District Magistrate or First Class Magistrate

If the applicant is applying as a grandchild of an Indian National, then the


applicant must submit his/her birth certificate as well as his/her parents’ birth
certificate. All submitted birth certificates must show the parents’ names clearly
so that it establishes the relationship with the applicants parents’ and
grandparents’, AND
(any One of the below documents)
- Copies of first three and last two pages of BOTH parents’/grandparents’
Indian passport, OR
- Copy of BOTH parents’/grandparents’ domicile certificate issued by the
competent authority, OR
- Copy of BOTH parents’ OCI cards/ grandparents’ OCI cards including
information pages on OCI cards.
Note: If parents’/grandparents’ have OCI/PIO cards, it is mandatory to give copies
of BOTH - Indian Passport & OCI/PIO Cards.
11 Naturalization Certificate (if Naturalized Citizen) - (ONLY IF APPLYING FOR THE NEW OCI) Copy
- If you acquired US Nationality through Naturalization, a copy of your Naturalization
Certificate is required.
- If you do not have an Indian passport and are applying for OCI on the basis that the
parents and/or grandparents were Indian nationals – then PARENTS AND/OR
GRANDPARENTS NATURALIZATION CERTIFICATE IS NEEDED.
- If the applicant has more than one NATIONALITY after having INDIAN Citizenship,
please provide all NATURALIZATION CERTIFICATES or PROOF OF NATIONALITY from all
countries where the applicant has held citizenship.
- The US Naturalization Certificate should state “FORMER NATIONALITY” AS INDIA
Certificates that state ‘Country of Birth’ as India CANNOT be accepted.
- If you gained naturalization through your parents and possess no separate
naturalization certificate, then copies of the parents naturalization certificates and a
Notarized statement signed by BOTH parents is required, stating the circumstances in
which their child received his/her naturalization status and why the child does not
possess his/her own certificate.

If Notarized copy is not available, then a Certified True copy from local USCIS office must
be provided.

If your name in any of the supporting documents provided is different in any way from the
name in the US PASSPORT, a copy of ONE of the Name Change Documentation must be
provided
- Marriage Certificate showing the name change if the name was changed at marriage

- Official Court Order for the name change
If in case the middle name was changed to an initial then a notarized statement letter is
required
17 Birth Certificate (if born outside India) must reflect following: Copy
- Parents' Name
- Date of Birth
- Nationality
If the applicant is applying through parents, and one parent was born outside India, then
please provide the Birth Certificate and Passport copy of the parent born outside India.
Birth Certificate has to be apostilled.
If the BIRTH CERTIFICATE/Documents are in any other language besides English, the
document needs to be translated into English and it has to be notarized.

25 Apostilled Marriage Certificate is mandatory in case you are including spouse name in Copy
your OCI Card – Applicable for all OCI Categories.
- If you are Divorced: Copy of your Divorce Decree - Applicable for all OCI MISC and OCI in
Lieu of PIO.

- If you are Divorced and Re-Married: Copy of Divorce Degree (All Pages) and Copy of
Current Marriage Certificate - Applicable for all OCI MISC and OCI in Lieu of PIO.

- If your Spouse is Deceased: Copy of Spouse Death Certificate - Applicable for all OCI
MISC and OCI in Lieu of PIO.
26 Proof of Particulars to be Changed (IN CASE OF OCI MISC. AND OCI IN LIEU OF PIO) Copy
Address
- Copy of documents in support of the Change in Address of the applicant.
For example - if shifted to a New Place, then copies of utility bill/s as proof of address
with the current address mentioned is required.

Appearance
- Copy of documents in support of the Change in Appearance of the applicant.
For example - if appearance of the applicant has changed from the past look, then a
new photograph reflecting the new look/appearance is required.

Occupation
- Copy of documents in support of the Change in Occupation of the applicant.
For example - if changed Occupation, then copies of documentary evidence is
required.
Change your Name
- Copy of documents in support of the Change of Name of the applicant.
For example - if the name of the applicant has to be changed, documentary evidence
for effecting the requisite changes is required.

Change your Signature


- Copy of documents in support of the Change of Signature of the applicant.
For example - if the Signature of the applicant has to be changed, documentary
evidence for effecting the requisite changes is required

Date of birth
- Copy of documents in support of the Correction in Date of Birth of the applicant.
For example - if the Date of Birth of the applicant has to be corrected, documentary
evidence for effecting the requisite changes is required.

Place of birth
- Copy of documents in support of the Correction in Place of Birth of the
applicant.
For example - if the Place of Birth of the applicant has to be corrected, documentary
evidence for effecting the requisite changes is required.

Spelling in Father's name


- Copy of documents in support of the Correction of Spelling in Father’s name of the
applicant.
For example - if the Father’s name of the applicant has to be corrected, documentary
evidence for effecting the requisite changes is required.

Spelling in Mother's name


- Copy of documents in support of the Correction of Spelling in Mother’s name of the
applicant.
For example - if the Mother’s name of the applicant has to be corrected, documentary
evidence for effecting the requisite changes is required.

Split of Name
- Copy of documents in support of the Split of Name of the applicant.
For example - if the name of the applicant has to be split, documentary evidence
for effecting the requisite changes is required.

Passport Particulars
- Copy of documents in support of the Change in Passport particulars of the applicant.
For example - if the passport particulars of the applicant have to be corrected,
documentary evidence for effecting the requisite changes is required.

Addition of Spouse Name


- Copy of documents in support of the Addition of Spouse Name of the applicant.
For example - if the Spouse’s name of the applicant has to be added, documentary
evidence for effecting the requisite changes is required. (Marriage Certificate)

Addition of Mother’s Name


- Copy of documents in support of the Addition of Mother’s Name of the applicant.
For example - if the Mother’s name of the applicant has to be added, documentary
evidence for effecting the requisite changes is required. (Birth Certificate)

 Indian Embassy/Consulate may request for any additional document as and when needed during the
OCI application process and may also call the applicant for a personal Interview.
 In case of foreign spouse, it is mandatory for the foreign spouse and Indian spouse to appear at the
Indian Embassy/Consulate for a personal Interview.
 Applications with incorrect details on the OCI government form will fall under ineligible category and
shall be rejected immediately. In this case, applicant would have to restart the entire process again
since the old application will be voided.
 Application with lack of documents will be considered incomplete and will remain on hold for 21
days. On none receipt of the missing documents, application shall be Abandoned and returned back
to the applicant. In this case, applicant would have to restart the entire process again since the old
application will be voided.
 Only Consular fee + ICWF fee will be refunded back to the applicant in case the application is
rejected or abandoned
