Abstract
Waham is a reality orientation disorder caused by impaired brain function. This study
aims to provide a description of implementation of nursing care to mental disorders
patients with major problems of waham at RSJ Radjiman Wediodiningrat Lawang.
This research is a case study emphasizing on interviews and observations and using
descriptive methods studied qualitatively. From the results of the assessment, it was
found that the process of the emergence of the problem has been in accordance with
Stuart's theory. It states that the problem arises due to the predisposing factors. In this
case, it is bankruptcy experienced by the client so that it shows maladaptive response
after being studied, it is obtained nursing diagnoses called changing of the main
waham thinking process, sleep pattern disorders, and spiritual distress. Interventions
are done from SP1 to SP4 with the implementations begun from assisting reality
orientation, assisting to fulfill the unfulfilled needs, training of their own capabilities,
giving education of how to use drug, and evaluating activities that have been
implemented. Activities can be done for 6 days; SP1 can be done for 3 days, SP2
done for 1 day, SP3 for 1 day and SP4 also for 1 day. From the results of this study, it
is expected that nurses will not only rely on medical treatment, but rather apply
nursing interventions in accordance with the established implementation strategy.
