PTA COMMITMENT CHECKLIST

I0. For all officers

I, _ resident of _ is filing my candidacy in general pta election of LIGAO NATIONAL HIGH SCHOOL
PARENTS AND TEACHERS ASSOCIATION for school year 2019 - 2020 and after an honest evaluation of
myself, i found that i satisfied the following conditions/requirements set by the association:

A) I am the biological mother/father of _/I am a court-appointed guardian of _

B) CAWIT which means that

- i will attend the LETRAP on June 15, 2019 from 8 :00 AM to 5:00PM to make me Competent/capable as
a GPTA officer

- i am Available every 2nd Sat of the month at 2:00 pm for the regular meeting and during special
meetings.

- i am Willing to render service free of charge and not expecting anything in return nor a special privilege

- i will Initiate projects in coordination with my fellow officers and the PTA adviser

- i will be Transparent in my transactions related to PTA.

C) I will voluntarily resign as an officer after incurring 3 absences during regular and special meetings

D) I have not been a general PTA officer for the last 2 school years (sy 2017-'18 and 2018-'19)

E) I am awared that GPTA officera are prohibited from participating as bidder in any PTA project.

GPTA TREASURER'S COMMITMENT

I.....

do hereby swear that i will report to the school every Monday from August to Feb and twice or thrice a
week in March , for purposes of accepting payment from the parents/students

That I am authorized to collect and issue receipt in the LNHS PTA office only.

That I will keep the collections in the volt for safekeeping and immediately deposit it in the bank the
suceeding day.
That if I will be replaced once i fail to discharge my functions for 3 times.

That i am accountable for the receipts issued to me by the association.

That I will always present an audited report of collections and disbursements every regular meeting.
Unliquidated expenses shld also be reported every meeting.

That i am entitled to handle and manage a petty cash amounting to 5 thousand pesos.

That i can claim for replenishment of any amount spent.

That I shld submit my report to GPTA Auditor monthly for auditing purposes which in turn will be
submitted to an external auditor/bookkeeper the services of which will be paid from the PTA collection.

Auditing by an external auditor or bookkeeper shall be done two times a year, one in October and
another after the Brigada.

Strictly, no.liquidation, no release of budget

GPTA AUDITOR'S COMMITMENT

I.....

That i will audit the treasurer after every collection & disbursement of the GPTA Treasurer.

That the audited report shall be presented every regular meeting of the GPTA

Moreover, i am aware that the audited report will be submitted to an external auditor/bookeeper two
times a year, that is in Oct and after Brigada.
Further, the GPTA officers have the discretion to select the external auditor who have established
credibility in line with his/her work as auditor.
