This is to certify that the vacation leave of JERWIN OCAMPO MACASINAG, Teacher- I/Junior
High School Teacher of Milagrosa National High School – Sorsogon Division under Regular Permanent
Status, on February 28-March 5, 2019 will not hamper the operational efficiency of the agency. Rest
assured that all works primarily assigned to him are made beforehand for the continuous productivity of
the office. Furthermore, an immediate follow-up will be made to his superiors upon returning to work.

This certifies further that HAINA BIO – Teacher I, JECILLE PORTUGAL Teacher –I and ELIZABETH
MACASINAG Teacher II, shall take over/assume the functions of Macasinag during the above stated
period.
