AFFIDAVIT OF LOSS
I, VINCE DANIEL T. ROJAS, 17 years old, single, Filipino, a resident of
Pasonanca, Zambonaga City after having been subscribed to an oath, hereby depose and
say THAT:

1. I am a holder of a school identification card, issued by the Zamboanga City State

Polytechnic College with ID No. 18-00247;

2. I used to place said ID inside my bag every after school for safekeeping;

3. However, sometime last February 19, 2020 when I arrived home, I discovered that
my ID was missing;

4. All my efforts to recover said ID were in vain;

5. I am executing this affidavit for the purpose of affirming the truth of the foregoing
statements and for any legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 9th day of March
