depose and state that:

I am the complainant in the foregoing administrative complaint

against PO2 Cardo M. Dalisay of Police Station 2 Cotabato City.

I caused the preparation of the complaint, which I have fully

read and understood.

I hereby affirm that all factual averments contained in the said

complaint are true and correct of my own personal knowledge and
belief, as well as true and correct on the basis of authentic
documents and records in my possession.

I certify that other than the criminal complaint I have instituted

for violation of Section 4 (a) of Republic Act No. 9995 against
respondent, which is docketed as Criminal Case No. xxxx at the
Regional Trial Court Branch 27 in Cotabato City, I have not
heretofore commenced any other action or filed any claim involving
the same issues in any court, tribunal, or quasi-judicial agency.

If I should hereafter learn that any other similar action or

petition has been filed or is pending, I shall report that fact within five
(5) days from knowledge thereof to this Honorable Court.
