AFFIDAVIT OF RELATIONSHIP

I, JUVY ANN VIOYA NOPRADA, of legal age, single, and with address at San
Jose, Gerona, Tarlac, after having been duly sworn in accordance with the law, do hereby
depose and say:

1. That I have been in a relationship with JOHN VICK BUTTERFIELD for

almost 6 years now;

2. That we met back in 2012 and after a few months of being friends, we
officially became a couple;

3. That since last year, we agreed to live together in one house and now we
settled in San Jose, Gerona, Tarlac;

4. That this second week of December 2018, we will get married in Republic of
Palau;

5. That we are binding into marriage in our own free will and with no intention
of fraud and defamation;

6. That this affidavit is being made by me for the purpose of assuring that we
have a true and loving relationship that was entered in "good faith," and is not
for the purpose of circumventing laws.

7. That I am executing this affidavit to attest the truth of the foregoing above
facts and for whatsoever legal purpose it may serve.
