state is good, the last vital signs were TD: 120/80 mmHg, Temperature 36,8 c, HR:
71X/mins , RR 20x / i GCS 15. Patient Complains with breast pain. Nursing Problems
are discomfort and, pain. The pain was on 4 scale. The interventions that already
done are checked the vital signs, the relaxation and distraction, the injection of
tramadol 1 amp, the injection of cefotaxim 500mg. Please keep to monitoring the
vital signs and the pain scale. Then, don’t forget to give the the next injections of
tramadol, cefotaxim, oh and then the infusion is on NaCl 20 drips/mins. Doctor
already visited this morning.

PA 3 : Mr. Santo, minimal dependency level too, 50 years old, with post laparatomi 7th
day. The vital signs were TD: 110/70 mmHg, Temperature 36,5 c, HR: 64X/mins , RR
20x /mins. There’s no complain from him. The pain is on 2 scale but he said he can
control is by himself without any medication needed. The doctor said he can go
home today, so nurse Benni can prepare for his things today for the checkout.
