The Span 1 concrete surface has a longitudinal crack (0.125" W x5" L) and delamination (1” Wx 17-6" L). Smaller diagonal eracks (0.06" x 1” L) are also stemming out from the longitudinal crack. These deficiencies are located approximately 10” from the west curb and 15" from the Abutment 1 joint. D2. Approximately 1" of the compression seal is missing at west curb line, with 3° additional tength ripped/tom for the expansion joint at ‘Abutment 1 D3. The south bracket on the sign attachment is pulled away from the fascla approximately 0.75”. All four bolts are rusted and the top bolt (away from midspan) is broken off. D4. The Span 2 scupper, south of Pier 2 on the west curb, has a damaged grate. There is a broken bar and the grate rocks under load. This deficiency is considered a critical finding. 5. There are no noteworthy deficiencies for the Span 2 top cancrete surface. 6. Thete are no noteworthy deficiencies for the Span 3 top concrete surface. D7. The west fascia edge of deck exhibits spalling (1" H x 3° Lx 0.5" D) centered about 12 south of Pier 3, Rust staining is present, but there is no exposed rebar. The overhang soffit exhibits transverse cracking (0.187" W x 4" L) at approximately 1" crack spacing over 24" L of the span starting from Pier 3, D8. There are no noteworthy deficiencies for the Span 4 top concrete surface. D9. The Span 1 and 2 deck soffit between Girders 2 and 3 (75’ L x 61-6" W) exhibits transverse cracking (between 0.125" ard 0.187" W) al 1" Wu 2° crack spacing. 10. Traffic safety features: No signs of collision damage on transition, guard rail or end treatment. 11, The 4* railing panel at Span 3, west parapet, exhibits collision damage. The Span 2, Girder 6 web-diaphragm connection above Pier 2 at the ‘channel beam top flange weld exhibits cracking (0.75” long in the top, bottom, and end return), ‘5p2. The lower half of the web of Girder 6 exhibits section loss (8" L) at approximately 3” from the girder end near Abutment 1. The remaining thickness measured 0.18”. The original thickness of the web was 0.25", p3. The rocker bearing is tilted excessively in the expanded position and exhibits heavy surface rust at Girder 6, Abutment 1. Heavy dirt accumulation is present under the rocker bearing. This deficiency (excessive tilt) is considered a critical finding Sp4. The field splice at Girder 4, Span 3 has one bolt missing in the web splice plate. The missing bott is located 4th from the bottom on the southerly row. The field splice also exhibits heavy rust bleeding Hammer sounding suggests that the field splice ts possibly loose from the bottom row through the fifth from bottom row. The length of the field splice flange plate Is approximately 2°-6”. ‘Sp5. Sloppy intermittent weld on the utility bracket connection to the diaphragm, between Girders 1 and 2. There were no visible cracks detected at the ends of the welds, Sp6. The protective coating system (paint) for the girders isin fair condition with surface dulling (20% of total area) and poor condition with toss of. pigment (20% of total area) excluding Deficiency Sp2, where the protective coating has failed. The protective coating system for the bearings is in fair condition with surface dulling (30% of area), ‘excluding Deficiency Sp3. Substructure: Yel No ‘Sb1. The west column at Pier 2 exhibits spalling (1°-8" W x 2'-5” Hx 2” D) beginning approximately 3” above the ground line. Additionally, 4 ties and 3 vertical rebar are exposed at this location. There is no section logs on the rehars Sb2. Three flexural cracks (0.06” W) are present at the Pier 2 concrete pier cap, between the first and second columns (Girders 3 and 4). All three cracks extend across the entire bottom face of the cap. The west crack continues 1°-6" up the vertical face (south face) of the pier cap, While the middle crack continues 2’ up the vertical face (south face). ‘The east crack is not present on the vertical faces of the pier cap. This is located on the Bent 4 pile cap in the cantilever on the downstream side of the bent. The crack is along the bottom ofthe pile cap and extends at a 45 degree angle 1-3" up both sides ofthe cap. The outer pile at the upstream side at Bent 1 is distorted and split. The spit starts 1” above the current water level, ends 2 above that, and measures approximately 5" wide. Decay and section loss are present. The decayed area is 3-2" in length by 1 wide, ‘The outer pile at the downstream side of Bent 1 is decayed 1-4" above and 1" below the current water level. The undamaged portion of the pile measures 18" in diameter and the portion of the pile with decay measures approximately 15" in diameter. ‘The outer pile at the upstream side of Bent 2, has a concrete ‘encasement retrofit 5-6" above the bed. The concrete ‘encasement was installed since the last inspection. The timber pile above the retrofit isin good condition. ‘The abutment with timber piles displays an exposed abutment face with soil scoured away and exposed embankment below the lagging, This is located at Abutment 1 in the center of the ‘abutment to the upstream comer.
