Annex “A”
CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:

This contract made and executed by and between the Sorsogon 1st Engineering District, Department of Public
Works and Highways, represented by Engr. IGNACIO T. ODIAMAN District Engineer, hereinafter called the FIRST PARTY
and Mr./Ms. Anita D. Janaban et. Al , Filipino, of legal age and with postal address of item 1-5, all in Sorsogon City,
hereinafter called the SECOND PARTY;

WITNESSETH THAT:

1. The FIRST PARTY is in need of the service of the SECOND PARTY who shall perform work not performed by the regular
personnel of the FIRST PARTY.

2. The SECOND PARTY has signified his intention to which the FIRST PARTY has accepted, to provide the services needed
by the latter;

3. The SECOND PARTY hereby possess the education, experience and skills required to perform the job as described herein;

4. The SECOND PARTY hereby attests that he/she is not related within the third degree of consanguinity of affinity to the
hiring authority and that he/she not been previously dismissed form government service by reason of administrative
offense;

5. In view hereof, the SECOND PARTY is hereby contracted as Administrative Aide I effective NOVEMBER 3 –
DECEMBER 29, 2014, with a compensation of the daily rate of Four Hundred NinePesos (Php 409.00) to be paid on
a 15-30 basis, assigned at the Administrative Section.

6. As Administrative Aide I, the SECOND PARTY is expected to performed the following functions:

ITEMS 1-5

1. Prepare monthly remittances, reports of disbursement number payrolls & vouchers, prepare annual BIR, income tax
return, withholding tax and does other miscellaneous office works and D.A. reports.

2. Type report, check cashbooks, submit RCI, RC, RD, receiving and filing incoming communication, payrolls, maintain
cleanliness at the COA office and check submitted liquidation report of Trust Fund D.A.

3. Prepare monthly reports of checks issued for Fund 101 current LDDAP ADA, Fund 151, MVUC external, 151 LDDAP
ADA, Trust Fund D.A. UCPB/LBP, prepare liquidation reports of D.A., and update cashbooks for Fund 101, LDDAP,
151, Trust Fund and national collection.

4. Record Purchase Request and Purchase Order. Check and balances the received supplies and materials. Prepare
issuances of various supplies. Record the approved Fuel issuances Monthly Consumption with details and complete
documents.

5. Maintain cleanliness of offices of various sections and comfort room.

7. The SECOND PARTY shall perform work from 8:00 a.m. to 5:00 p.m. (Monday-Friday) or any agreed schedule
provided it does not exceed 40hrs. per week or 22 days per month; and

8. It is understood that this contract does not create an employer-employee relationship between the FIRST PARTY and the
SECOND PARTY, that the services rendered is not considered and cannot be accredited as government service; and that
the latter is not entitled benefits accorded to regular personnel of the FIRST PARTY.
