2 Scope of Works/Brief Description of Project/Key Hazards


Scope:
1. Erection of 45 Mtr. AD9090 tower at ____________with reference to the
drawings, Tower Key diagram, Panels assembly diagrams and platforms
diagrams.

7 Environmental aspects and weather
No Hazardous substances will be used
Only licensed and certified cranes and machinery will be used hence
environmental impact (e.g. pollution) shall adhere to local laws
Any rubbish items shall be cleared away from site upon completion of the job
8 General Safety
Adequate prevention measures will be taken to prevent access to site from
members of the public. Unauthorized persons will not be permitted on site.

Safety helmets must be worn at all times during lifting operations and whenever
there is work going on overhead.
 
Safety shoes must be worn at all times on site.
 
All tools must be visually inspected prior to use to ensure that the leads and

equipment are in good working order.


 
All personnel are to ensure that they are in a fit state to work at all times. If they
are not, they must cease work and rest until such time as they are.

When handling galvanized steelwork or fibre glass, operatives should check for
sharp edges and remove as necessary. Gloves should be worn when handling any
steelwork.
 
All manual handling must only be carried out in line with training and
recommended lifting weights should not be exceeded.
 
In the event of adverse weather work should stop until such a time as the
supervisor feels it safe to continue.

First aid kits will be provided to the team. A trained first aid assistant will be made
available to the team.

Fire extinguishers will be provided to the team. A trained fire-fighter will be made
available to the team.

Prior to vehicles accessing site, the site supervisor is to ensure that there are no
new hazards, and that the access roads are safe. Further, he is to ensure safe
location for the crane and delivery vehicles.

Any unsafe acts or incidents will be dealt with immediately. The violator will be
informed of the violation via the Safety Violation Notification Form, and if
necessary, will be asked to leave from duty.

Any near-misses or accidents will be reported to the Project Manager and/or the
Safety Officer, and recorded in the near-miss registration form.
9 Vehicle Movements
A designated area shall be determined for parking
A designated Banksman will be appointed for control of vehicle movements

11 Pre-Start Meeting
1. Site supervisor to determine safe location for cranes, delivery truck, vehicles
and site material placement.
2. Supervisor to also determine safe access and egress to site.
3. Assemble team and conduct toolbox talk (covering the risk assessment and
this method statement).
4. Supervisor will ensure everyone is aware of the safe system of work and
obtain signature of attendees during toolbox talk.
5. Site team will set up barrier tape and cordon off the site from members of
the public. A warning sign will also be posted.

12 Works Method
1. The tower consists of several panels (C, D, E, F, G, H, J, K, and L), panel C
being the topmost and panel L being the lowest.

Assembly and installation of panels L thru J

2. The site team will start with the lowermost panel (Panel L).
3. The 4 sides of the panel (legs and crossbeams) will be assembled
individually.
4. The supervisor will check if holding down bolts and leveling nuts are ready
for installation.
5. An exclusion zone will be cordoned off with barrier tape and cones. The area
will be checked by the supervisor to make sure all personnel are in position
and no unauthorized personnel are within the lifting or drop zones and the
crane will be moved into position.
6. The supervisor will fill out “Safe Use of Crane” checklist – ensuring that
crane, load and lifting gear are suitable and sufficient.
7. A Banksman will be assigned by the supervisor to control crane movement.
The supervisor will ensure the crane operator is aware who is the Banksman
and will ensure the operator pays attention only to him.
8. In each case the lifting chains/slings will be adjusted to ensure that the load
remains level during the lifting operation and the slings are operating within
their safe working load making due allowance for the included angle.
9. A rope tail line will be attached from a ground anchor to the bottom of the
load to ensure the load does not swing. The rope will be attached using a
friction type slip knot. This will be loosened appropriately as the lift is raised.
10. One of the panel sides will be lifted and positioned onto the holding down
bolts and the leveling nuts.
11. Once the panel side is sitting correctly on the leveling nuts, top side holding
down nuts will be secured with lock nuts.
12. Once the side is secured, 2 riggers will don appropriate fall protection gear
and climb up the panel side and release the slings of the crane.
13. This process will be repeated for the other 3 sides of the panel.
14. Once the entire panel is secured, the internal structures (e.g. work
platforms) will be lifted into place and secured appropriately as indicated in
the drawings.
15. Once the supervisor is satisfied that Panel L is secured, 2 additional riggers
will don their fall protection gear and climb up so as to have 1 rigger on each
corner of the tower. They will secure themselves and await the lift of the
next highest panel.
16. The next highest panel (Panel K) will be assembled in a similar fashion to
Panel L.
17. Each side of Panel K will be lifted individually and secured by the riggers on
the appropriate corners using the appropriate plates and splice bolts.
18. Once Panel K is secured, the riggers will move to the top most part of Panel
K and the installation of the next highest panel (Panel J) will start in exactly
the same way until Panel J is completed.

Assembly and Installation of panels H thru C

19. Panel H thru C do need individual assemblies of the panel sides. Instead, the
panels will be assembled completely on the ground as one box unit and lifted
as a whole.

20. Panel H will be assembled in its entirety and lifted by the crane and
positioned over panel J.
21. The riggers will, as before, secure the panel using splice bolts.
22. Any internal structures will be lifted and positioned into place where
indicated in the drawing.
23. The next highest panel will be assembled and installed in exactly the same
way until Panel A is completed.

Installation of ancillary equipment

24. Once all panels are successfully secured, the riggers will attach a rope and
pulley system on the tower.
25. Riggers will then hoist the ancillary equipment such as ACWL’s, lightning
spikes, radar / camera mounts, one by one as indicated in the drawings.
26. They will then install and secure them in the appropriate location on the
tower as indicated in the drawings.
27. Once installation is complete, supervisor will climb the tower (wearing
appropriate fall arrest PPE) and inspect the tower and its installation.
28. Once supervisor is satisfied, touch-up painting works shall be carried out.
13 Completion
1. The crane will be packed up and moved off of the site.
2. All lifting gear, tools, and fall protection gear will be inspected after use for
any severe wear/tear. Any defaults will be reported to the appropriate
manager.
3. A site H&S checklist will be filled in by the supervisor to report on the day’s
activities.
4. all site debris, rubbish and other materials from the site will be cleaned up.

AFTER COMPLETION OF WORKS**
Prior to commencing the operation all site personnel are to sign below to confirm that a
clear briefing explaining the job has been given and is understood, along with a brief of
the risk assessment pertaining to the site.
Any concerns raised during toolbox talk should be recorded on this form and
reported to concerned Project Engineer/Manager or to the Safety Officer.
