She goes above and beyond for her clients. She came in on a weekend to help out with a client party because a
client wanted her to watch her play the bells. Alyssa places her client's needs and even desires above her own
self interests.

independently. In the beginning she was appreciative of feedback. She does consistently ask for feedback from
clients to be sure she is addressing their needs correctly. She understands the importance of evidence based
treatment and has demonstrated she is able to locate and interpret helpful research.

correctly. She uses her time wisely and reviews any medical history she can access before the client arrives. She
is able to independently change the tests she is using when the client presents different than what was expected.

limitation. She is able to correctly predict correct prognosis for most clients, and is comfortable with the fact
that in this population it is not always possible to tell immediately.

correctly assesses client's response to intervention. She has not participated in any quality/performance
improvement programs- so this is a potential goal for her next setting.

CI Mid-Term — Michelle K. Klaty ALyssa uses her time effectively and is willing to stay after to be sure patients are seen. She does well following
up with patients when they are late and rescheduling with supervision.

CI Final — Michelle K. Klaty Alyssa is flexible with her time so she can see as many clients as possible, by adjusting schedules. If a client
cancels she is independent in calling to see if another client could fill the space giving the canceled client an
opportunity to come at a later time. Alyssa uses her time and company resources wisely.

CI Mid-Term — Michelle K. Klaty Alyssa does very well communicating with our PTA and legally, ethically and appropriately delegates.

CI Final — Michelle K. Klaty Alyssa does a great job correctly directing staff with transfers, HEPs and safety with walkers and wheelchairs.
She also does a great job independently and respectfully transferring clients over to PTA and overseeing the
care provided by the PTA.

Areas of Strength
CI Mid-Term — Michelle K. Klaty Alyssa is very timely, hard working and trustworthy. She easily establishes rapport with all of our patients as
well as other clinicians. She evaluates patients, treats patients and presents in team meetings accurately and it
appears easily.

CI Final — Michelle K. Klaty Alyssa is extremely dependable- she shows up early, completes tasks in a timely manner without being
reminded, and goes above and beyond what she says she will do. Alyssa is very compassionate with her clients.
She shows empathy and treats clients with respect- even the more difficult clients love her. Alyssa does a good
job with her treatment plans, over time I am sure her variety will improve even more. Alyssa is currently
performing as an entry level PT and I would not hesitate to hire her.

Areas for Further Development

CI Mid-Term — Michelle K. Klaty Communicate alternative treatments. Discuss evidence for treatments and measurements.

CI Final — Michelle K. Klaty Alyssa could improve on her confidence. I am sure this will come with experience, but she consistently provides
great treatment and stays aware of possible safety concerns and alerts the correct people when needed.

2) Considering the rating anchors, what percent of a new graduate caseload is the student capable of managing at this time?
CI Mid-Term — Michelle K. Klaty Seven (7) clients per day. Alyssa is close to 100% with supervision.

CI Final — Michelle K. Klaty 7 clients/day Alyssa maintains a full caseload.

Recommendations

CI Mid-Term — Michelle K. Klaty Continue to do great. As stated above discuss treatments and evidence to support. You are doing a great job and
make things appear effortless.

CI Final — Michelle K. Klaty Continue to be a lifelong learner. Continue to strive for excellence, you are an assist to the field of physical
