Appendix A: Daily Reflective Diary
Date/Rotation: 30 Jan 2012 (day 1)/Care of elderly
Today’s activities.
 Met with Ms.Shuhaidah, our preceptor for care of elderly rotation.
 Briefing by our preceptor about the rotation of care of elderly.
 Clerked a case in ward D3 involved a 76 years old patient (ABH), with hypertension, congestive heart failure, atrial
fibrillation and dignosed with AEBA secondary to community acquired pneumonia. ABH was administered to the
hospital due to shortness of breath and fever
A significant experience/observation today.
 Observation on the management of AEBA secondary to community acquired pneumonia

Learning from this experience/observation.

1. Management of AEBA
 According to Brithish Asthma Guideline, nebuliser Combivent® 4hourly and Pulmicort® 1000mg BD, intravenous
hydrocortisone and tablet prednisolone should be prescribed to the patient. Tablet prednisolone will be stopped after start
with MDI budesonide. MDI salbutamol 2puff prn and MDI budesonide 2 puff BD was prescribed to the patient.
Sustained release theophylline 250mg BD was prescribed to the patient if the treatment still not adequate.

Additional learning needs from this experience/observation and my plan to achieve the learning.

1. Management of community acquired pneumonia


 According to National Antibiotic Guideline 2008, azithromycin 500mg IV/PO OD OR erythromycin 500mg IV QID or
erythromycin 800mg PO BD PLUS 3rd gen. Cephalosporins,e.g. Ceftriaxone 1-2g IV OD OR Augmentin® 1.2g tds for
1 week should be prescribed to the patient

Summary of what I have learnt.
