TRANSFER ACTION TEMPLATE

REGULAR TITLE
DOCUMENTARY REQUIREMENTS:
 The list of documentary requirements does not preclude this office from asking additional documents and conducting
other processes in relation to the mandate of Republic Act 6657 as amended by Republic Act 9700, and
Administrative Order No. 1, Series of 1989.
 The application must be three (3) sets and fastened in folders: the 1st Folder is Original or True Copies and 2nd and 3rd
Folders are Photocopies.
 The documentary requirements must be in the following order:

___1 Written request signed by the applicant: transferor, transferee, or any of their authorized representative;
.
___2 Deed or instrument to be registered (Donation, EJS of Estate, Sale, Waiver);
.
___3 Certified true copy of the OCT/TCT and/or Tax Declaration if the land is not covered by a Certificate of Title;
.
___4 Affidavit of the transferee that he/she and spouse have a total landholding inclusive of the land to be acquired of
. not more than five (5) hectares. Copy of the affidavit shall be furnished the BARC with proof of service indicated
or attached to the copy for the Register of Deeds;
___5 Affidavit of transferors stating that the land subject of deed is a retention or portion of the retention area.

For Extrajudicial Settlement of Estate and Affidavit of Self-Adjudication please take note:
___6 A death certificate must be submitted as additional documentary requirement;
