Items to be Included
1. Filled Organizing Committee List (with full details listed)
2. Original Receipts
● Expenses without receipts must attach (23RD) INTIMA Memorandum-SSD for JUSTIFICATION
with signatures of INTIMA EXCO and Manager of SSD.
● Invoices, photocopy receipts and banking slip will not be accepted.
● Phone Reload Cards (e.g. HOTLINK Prepaid) will not be accepted.
● Receipts attached SHOULD NOT BE FOLDED.
3. 10 black & white photos

● Email the 10 pictures to jeremyteo.intima@gmail.com with Event Name as Subject.
4. Participant List (with full details listed)

● Must be written and signed by participants.
● Participants with no full details WILL NOT BE INCLUDED in total amount of participants for
INTIMA subsidy.
5. Liability Form
● Please obtain a copy of liability form IF the original copy is given to bus driver.

EVENT DETAILS
1 INTIMA EXCO In-Charge:
2 Activity Summary (min 200 words):
The program started at 7 pm with VIPs walk in. VIPs are the representative from INTIMA and Dr.
Owyong. The VIPs were invited to officiate the opening ceremony for the event. And also, the
organizing chairperson and club’s chairperson were invited to give a speech and presented the
souvenirs to the VIPs after the speech. Before starting the game section, participants have
already eaten dinner. After that, Emcee gave instruction to the participants about the game
rules and divided them into a different group. Some of the committees became the facilitator
and helper during the game section. Then, the games started. The game master has prepared
four types of game for the participants to enjoy it. We finished the game at 10 pm. After we
finished the game, we gathered all of the participants in the middle of the sports hall and having
a group sharing session. Some of them were sharing their experience on that night. They were
enjoying the games and happy to know many new friends. In the end, we took a big photo with
all the participants and the committee.

No.
8 What were the problems faced while organising this activity?
1) Unable to book the equipment and venue on time

2) Lack of the communication between the OC team
3) Hardly to find the sponsor
4) Didn’t expect the participants are many, making some change to the game
9 Did the activity achieve its objective?
Yes, the activity has ended up successfully with our theme which is Hawaii. All of the participants
are enjoying the moment of the game and foods.
10 Please give suggestions to improve the activity if it needs to be organized in the future.
1） Organizing team must be communicated well before every meeting.

2） Organizing team should have constant meeting time to have prepare everything well.
3） Every department must inform early to the secretary to book the equipment and venue.
4） Program Master should plan the program flow well and prepare for the unexpected
condition.
5） Pro could prepare some special ways or activities to promote the events.
6） Secretary should make sure everything that needs is under the booking.
