SCENARIO 1

A 65 years old woman came to the dentist with her son complaining the
incapability in chewing since she had loss all of her tooth. Intraoral examination
resulted that there were edentulous area in upper and lower jaw, with high
alveolar bone of the upper jaw, while alveolar bone of lower jaw has decreased.
Extraoral examination showed that there was angular cheilitis and short level of
face. The patient wanted to make a denture.

Topic : Full denture

- Difficult words: -
- Formulating and analyzing the problem
1. What is the definition of full denture?
Removable appliances used when all of the teeth within the jaw
have been lost and need to do prosthetically replaced.
2. What are the parts of full denture?
 base plate :
- polishing surface : surface that need to be polished
and will be contacted with foods
- fitting surface : surface that faces the mucous and
based on the anatomy.
 anasir tooth
 occlusal surface
 peripheral seal
 post dam : posterior retention in maxilla to
create vacuum area that prevent air from
entering the fitting surface.
3. What is the relation of angular cheilitis and the edentulous?
The edentulous will decrease the vertical dimension and will
cause lack of lip support. It will result to deep fold of skin at the
corner of the mouth which leads to collection of saliva in this area
resulting in skin becoming dry, fissured, and cause ulcers.
4. What are the functions of the full denture?
a. To restore masticatory function, vertical dimention, TMJ
b. Phonetic function
c. Esthetic function
5. What are the indications and contraindications of full denture?
Indication: edentulous
Contraindication:
- allergy to acrylic
- xerostomia
- abscess
- infected socket after tooth extraction
- patient with severe gag reflex (easy to vomit)
- excessive bone resorbtion
6. What are the anatomical landmarks of full denture? (Maxilla and
mandible)
Maxilla:
a. Papila insisiva
b. Ruggae palatine
c. Ridge
d. Tuberositas Maxilla
e. Hamular notch
f. Fovea palationa
g. Frenulum
h. Fornix

Mandible:

a. Frenulum
b. Ridge
c. Fornix
d. Retromolar pad

7. What are the types and materials of full denture?

- Material for base plate: heat cure acrylic
- Material for artificial teeth: resin acrylic or porcelain

Type of full dentures:

a. Metal denture base

b. Porcelain
c. Polymethyl metacrylate
d. Cad cam
8. How to make an impression for edentulous?
a. Prepare the materials (irreversible hydrocolloid or alginate)
and tools (Stock tray or custom tray)
b.
9. What is the vertical dimension and how to measure it?
VD divided by 2:
1. Psychologic rest position
2. interpuspa position (Occlusal vertical dimension)

VD measure method:

1. Praextraction notes
2. Leonardo Davinci
3. Willis
4. Nis Wonger (2 dots technique):
a. making the maxilla dot and mandible dot
b. the patient need to be in a rest position with saying the
word of M
c. Measure the distance
d. Inserting the record block in maxilla and mandible
e. Measure the distance
5. Physiologis
6. Phonation
10.How to do the extraoral and intraoral examination in edentulous
patient?
Extraoral examination:
1. Facial form (square, square tapering, tapering, and ovoid)
2. Facial profile (class 1, 2, or 3)
3. Lip examination
4. TMJ examination

Intraoral:

1. Arch size
2. Arch form
3. Ridge form
4. Inter ridge space
5. Ridge contour
