AFFIDAVIT OF NO OPERATION

I, FRANCIS BAGUIDUDOL, of legal age, married, Filipino citizen,

and residing at Piwong, Hingyon, Ifugao, under oath hereby depose and state:

1. THAT, I am the owner/manager of FBI Iron Work Welding Shop

located at Piwong, Hingyon, Ifugao ;

2. THAT, since the month of January 2015, the aforesaid business/shop

has ceased its operation and did not conduct any transaction of its
business until the present date;

3. THAT, said business has not earned any income from the date of non
– operation up to the present;

4. THAT, I am executing this affidavit to attest to the truth of the

foregoing and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I hereby set my hand this 2nd day of March
