Dear Atty. Baroña,

Good day! I am Kevin P. Belen, with the student no. 2014-89115, a student from the UP
College of Music taking DCPMA Major in Clarinet. I would like to apologize because I
honestly failed to attend all engagements of the UP ROTC Symphonic Band last semester
because of conflicts on schedule. I am also a Band Leader in our Province, San Pablo City,
Laguna and I am very sorry for I can’t make some changes in a glance on our schedule which
was long been scheduled.

I humbly ask your good office to still accept me to avail the ROTC Scholarship, for I may
not be able to graduate without it because I cannot pay the tuition fee without this scholarship. I
promise to be more responsible in all my duties for this semester and I’m very sorry for being
irresponsible last semester not thinking all the consequences I may face.

My reason may not be that acceptable but still I am hoping for your kind consideration
and promise that I will do better this semester.
