BENEFITS AND PRIVILEGES

Member and Spouse

1. Free board and lodging up 45days

2. Fifty percent (50%) discount on the following:

a. Professional fees based on house case rates and OPD Consultation

b. X-ray, UTZ, CT Scan, 2D Echo, Laboratory Procedures, ECG, EEG, Mammography, Tread Mill, Physical
Therapy

3. No operating room, delivery room and emergency room fees

4. 10% discount for pharmacy, CSR, OR materials.

5. For dental services:

a. Fifty percent (50%) discount on consultation

b. Thirty percent (30%) discount on Restorative, Preventive, Periodontics, Endodontics, Surgery and
Bleaching.

c. Ten percent (10%) discount on Prosthodontics procedures

6. For Medical Stockholders (after payment of PRIVILEGE TO PRACTICE)

a. May hold clinic within the hospital for a maximum of 6 hours a week paying only the maintenance,
water and electric bills; and

b. Will be included in the decking/rotation of House Cases

Member's Natural Parents and Dependents

1. Fifty percent (50%) discount on board and lodging

2. Fifty percent (50%) discount on the following:

a. Professional fees based on house case rates and OPD Consultation

b. Operating room, Delivery room fees, Emergency room fees

3. Twenty-five percent (25%) discount on the following routines:

a. X-ray, UTZ, CT Scan, 2D Echo, Laboratory Procedures, ECG, EEG, Mammography, Tread Mill, Physical
Therapy

4. Ten percent (10%) discount on Pharmacy, CSR, and OR materials

5. For dental services:

a. Fifty percent (50%) discount on consultation

b. Thirty percent (30%) discount on Restorative, Preventive, Periodontics, Endodontics, Surgery and
Bleaching

c. Ten percent (10%) discount on Prosthodontics procedureso

The above benefits shall be enjoyed for a duration of forty-five (45) days confinement per
Stockholder/Dependent per year, non-cumulative (no carry-over), non-transferable. After 45 days all
hospital charges will be at regular rate. For out-patients there is no limit to the availment of the
discounts.

The above privileges shall commence only upon full payment of shares of stocks subscribed, certified
and verified by the Corporate Secretary or his /her duly authzorized staff.

Every Member shall submit the names his/her legal spouse (if married) and dependents with the
following supporting documents: Marriage License (for married stockholders), Birth Certificate and such
other pertinent documents as may be required.

Health insurance benefits of whatever kind or nature, or other health-related entitlements under the
law, whenever applicable, shall be deducted first before determining the amount subject to discounts
under the above schedule of benefits.

Definition of Dependents:

• Legitimate or legally adopted single children under 21 years old

• Natural parents of Stockholder

The Privilege to Practice shall be paid by all medical stockholders. Start of practice will be from the date
of approval of the Credentials by the Credentials Committee.
