Dear Sir,

I have the honor to state that I am a student of your Basics in Natural Science course. I couldn’t

attend a few of my classes due to my sickness. Still I am recovering from sickness. In the

meantime, due to being absent from class I missed two quizzes. I have been suffering from joint

bone pain for about a month along with fever. I am attaching a doctor's prescription along with a

medical report photocopy herewith.

I, therefore, pray and hope that you will be kind enough to consider my problem & grant my

application for the said classes along with give me opportunity for makeup exam and oblige

thereby.
