Homan’s Test

Indications
Homan’s Test is an ankle examination procedure that tests for
thrombophlebitis and gastrosoleus strain. It is also called the dorsiflexion
sign and is used to assess for deep vein thrombosis.

Procedure
Patient should be awake and cooperative for Homan’s test.

1. Patient should be supine with the leg straight.


2. The examiner raises the patient’s straight leg to 10 degrees and forcefully
dorsiflexes the foot and squeezes the calf with the other hand. He may
also use a blood pressure cuff for this procedure.
3. The examiner checks for deep calf pain or persistent calf pain.

Interpretation
Positive Homan’s Test

 Short duration “deep” calf pain – thrombophlebitis

 Persistent “achy” calf pain – Gastrosoleus strain

Clinical Notes
Thrombophlebitis is also known as “intravascular coagulation” which refers
to inflammation and thrombosis of a vein due to local trauma, intravenous
infusion, hypercoaguable state or damage to the endothelial lining of the
veins all of which may ultimately result in venous blood stasis.

Moses’ Test
Indications
Moses’ Test is an ankle examination procedure that tests for vascular
insufficiency of the lower extremities as well as thrombophlebitis,
arteriosclerosis obliterans, gastrosoleus strain or MFTPs.

Procedure
Patient should be awake and cooperative during a Moses’ test.

1. Patient should be prone during this examination.


2. The patient should have her knee flexed 90 degrees.
3. The examiner squeezes the patient’s calf and observes patient for signs of
discomfort and pain.

Interpretation
Positive Moses’ Test

There is positive Moses’ test when there is

 Short duration “deep” calf pain – this could indicate lower extremity
vascular insufficiency, thrombophlebitis, arteriosclerosis obliterans.

 Persistent “achy” calf pain – gastrosoleus strain or MFTPs.

Clinical Notes
Patients with thrombophlebitis usually give a history of a gradual onset of
localized tenderness; there may be a prior history of local trauma, previous
similar episodes, varicose veins, prolonged travel or enforced stasis.
