Dear Sir/Madam;

This is to inform you that your contract at (insert name of barangay and municipality) with

exceeded five (5) days from the stated deadline in the contract. In lieu to this, delivered materials
after the deadline shall be subjected to liquidated damages which is equivalent to the amount of 1%
of everyday delay (weekends and holidays included). This will also apply to the remaining
undelivered materials listed below.
