Formative Assessments for Math Unit

Day 1 Formative Assessment:

The students will complete their making of their mammal when adding two two-digit numbers
together in order to find the sum. They then will take the sum and use centimeter graph paper
to model the answer. They will use the base-ten blocks then to create their mammal which was
a fox for day 1. They then will record on their “Base-Ten Picture Recording Sheet” their findings
of their mammal they created. A differentiated scoring guide was not used for this lesson, as
the students are using their knowledge that they gained when at the beginning of the lesson
when adding two two-digit numbers together.
Day 2 Formative Assessment:

The students will be now focusing on adding three two-digit numbers when given an equation
to find the sum. They then will be taking that sum and using centimeter graph paper to model
the answer. The mammals that they will be creating are a deer and a groundhog. They then will
fill out the “Base-Ten Recording Sheet in order to show understanding. A differentiated scoring
guide was not used. Student 1 will have to write in complete, detailed sentences when talking
about the mammal she created and student 2 will try and control his behavior throughout this
lesson.
Day 3:
The students will be now focusing on adding four two-digit numbers when given an equation to
find the sum. This is the last lesson of the sequence in order to meet the central focus of the
lesson which was being able to add four two-digit numbers when using place value strategies.
They then will be taking that sum and using centimeter graph paper to model the answer. The
mammals that they will be creating are an owl and a beaver. They then will fill out the “Base-
Ten Recording Sheet in order to show understanding. A differentiated scoring guide was not
used. Student 1 will have to write in complete, detailed sentences when talking about the
mammal she created and student 2 will try and control his behavior throughout this lesson.
