ADMISSION PROCEDURE

1. PURPOSE:

The purpose of admitting patient is to provide guidelines for routine admission in the

ward/unit for diagnostic/treatment from OPD or from Emergency.

2.SCOPE:

This policy is applicable for admission of all patients

3. RESPONSIBILITY:

3.1 Respective consultant

3.2 Staff Nurse in the Ward

3.3 Ward Secretary/ Floor Incharge

3.4 Resident

4.1 Admission is the entry process of a patient into a hospital ward for therapeutic or

diagnostic purpose

Types of admission

Emergency OPD.

4.3.1 Check the room for tidiness and completeness before receiving the patient

4.3.2 Greet the patient and relatives on arrival and introduce yourself.

4.3.3 Apply ID Band.

4.3.4 Check admission slip for completeness.

4.3.5 Check whether the admission is based on

a. Cash. b. Insurance. C. Medico Legal Cases

4.3.6 Check patient’s height and weight if the patient is able to stand

4.3.7 Accompany patient and relatives to the assigned room. Place patient comfortably

on bed and provide side rails.

If the patient is received from ER, take over all clear instruction from ER staff like

given medications, samples taken for investigation and other investigation like CT, X-RAY,

MRI and etc and collect all the reports from the concern department. Patient’s old file and

reports should be kept in patient case record.

4.3.7. Orient patient to the ward set-up and explain the call system, telephone, visiting

regulation and smoking restriction.

4.3.8. Check the vitals and record in the chart and complete the initial assessment form.

 The fall risk assessment to be done for all vulnerable patients.

 Explain patient and relatives not to give any home medicine in their own but

home medicines taken by patient must be brought to the attention of the attending

physician.

 Inform the Consultant and Resident and the Dietitian.

 Inform OT if the patient is posted for surgery

 The nurse should complete the admission notes and carry out all the orders.

 Get the signature from Primary Consultant on first visit of patient in the History and

Physical examination form.

 Record the patient’s detail in Nominal Register and in the White Board in the ward.
