temperatures, tornadoes
Adverse Environment Salt spray, industrial pollution, humidity, corrosion, vibration, fire, flooding
System Condition High and low voltage, switching, overcurrent, high and low frequency
Human Element Incorrect system records or diagrams; incorrect use of equipment, incorrect
construction, installation or maintenance; incorrect protection setting; switching
error; testing; incorrect circuit labelling; deliberate or accidental damage by
employees/contractors
Foreign Interference Contacts, deliberate or accidental damage o interference by persons other then
utility employees/contractors, vehicles, animals, trees, solar magnetic induction,
etc.
Unknown An outage cause could not be identified.
