Burden of disease
Mood disorders are among the leading causes of morbidity and
mortality in western and developing countries. This can obviously be
seen primarily as an accumulation of personal misery. Mood disorders
cause disability because they impair cognition and prevent people from
working for long periods of time. This causes absenteeism, which is
36 obvious, but patients with depression who return to work may be
emerge in the next few years. This is an area that demands patience
and caution because it is potentially highly controversial. If we make
diagnoses of bipolar disorder in very young children, we may be able
to treat them by extrapolating from adult experience. We may even
produce a better long-term outcome than would otherwise have been
possible. However, there are competing risks. If we use modified criteria
to make the diagnosis, how reliable can we expect it to be? How many
children do we treat ‘unnecessarily’ as a consequence? How good are
the data to ensure that the medical approaches we extrapolate from
adults are either safe or effective in children? Medical practice often
involves making unpalatable choices. This area is no exception.
