14.10.1998.

Through the reference 3rd cited above wherein the individual has submitted an
affidavit stating that her Date of birth was mentioned wrong in the said pass book and
requested to correct the date of birth as 12.05.1998 instead of 05.07.1998

Therefore while enclosing the relevant records the GCP final payment
proposals along with original pass book is submitted for further necessary action.
