AFFIDAVIT

resident of Brgy. ______________, after having been duly sworn to an oath
depose and say:

3. That recently I need his Baptismal Certificate, in connection with his

personal and official transactions;

4. That when I visited the Parish Office I found out that there was an
erroneous entry on his Baptismal Certificate, to wit:

Photocopy of his Certificate of Live Birth is attached herein as

ANNEX “A”;

the latter should be the correct entries, refers to one and the
same person;

7. That I am executing this affidavit in good faith to attest the

truthfulness of the foregoing statements in support with my desire
to correct his Baptismal Certificate and for whatever legal purpose
this may serve.
