SD due to oil tubing failure Cylinder #5

Event description
Date: January 15th.

Description: BBLDPE train was SD yesterday evening due to a leak detected on the lube oil tubing of
Hypercompresor cyl#5 (2nd stage), no gas leak detected. After inspections, a crack was found on this
tubing.
The plant SU after cylinder 4&5 corrective work and it was running for 2 days and a half when SD due
to a peroxide swap due to valve failure. Then it was SU again and it was running for 5 hours aprox until
the oil tubing failure.

After replace and inspect the check valve and tubing it was
seen high temperature signs due to the gas flow going
through the tubing.

During the hydraulic test it was confirmed the tubing leak

coming from the connection (thread) at 1500 Kg/cm2.

During the hydraulic test it was confirmed the check valve

leak at 5 Kg/cm2.
