GENERAL NOTES

1. All construction, materials and workmanship shall conform to City of


Lewisville Standard Details, latest edition, and Standard Specifications for
Public Works Construction, North Central Texas Council of Governments
(NCTCOG Standard Specifications), latest edition, unless otherwise
noted.

2. General construction sequence and traffic control is shown in the plans.


See Sheet __ for the sequence of construction. All traffic control must be
in accordance with TMUTCD latest edition. Contractor must notify property
owners 48 hours in advance of construction.

3. The Contractor shall provide temporary drainage measures during


construction.

4. The Contractor shall install all permanent pavement markings and


signage.

5. Bracing of utility poles may be required by utility companies when


trenching or excavation is in close proximity to the poles. The cost of
bracing poles will be borne by the Contractor. There is no separate pay
item for this work. The cost shall be considered incidental work included
in the Contract Unit Price bid for applicable pipe or structure installation.

6. The locations, elevations and dimensions of all existing utilities shown on


the plans were obtained from available utility company records and plans
and are considered approximate. It shall be the Contractor’s responsibility
to verify locations, elevations, and dimensions of adjacent and/or
conflicting utilities sufficiently in advance of construction in order that
adjustments can be made to provide adequate clearances. The
Contractor shall preserve and protect public utilities at all times during
construction. Any damage to utilities resulting from Contractor’s
operations shall be restored at his expense. The Engineer shall be
notified when proposed facility grades conflict with existing utility grades.

7. The Contractor shall immediately repair or replace any physical damage to


private property, including, but not limited to fences, walls, pavement,
grass, trees, planters and lawn sprinkler and irrigation systems at no cost
to the owner. This work shall be subsidiary to the contract, unless
otherwise noted, and is not a separate pay item.

8. The Contractor shall protect all trees in the project area during
construction. No tree shall be removed in the ROW unless marked and
approved for removal by the City Inspector.

9. All planters, vegetation, shrubs, etc. located within the right-of-way shall
be removed. The cost shall be incidental to the unit cost of “Prepare
Right-of-Way”.

10. During paving operations existing community mailboxes shall be removed


and temporarily reset in the parkway outside of proposed pavement
construction. After pavement construction, mailboxes shall then be moved
back to the original location.

11. The Contractor shall cut and plug all irrigation systems located within the
right-of-way prior to construction. After construction is complete, the
Contractor shall replace and reconnect the systems to original or better
condition, at no cost to the owner.

12. The Contractor shall remove surplus material from the project area. This
work shall be subsidiary to the contract and is not a separate pay item.

13. No onsite materials shall be used as topsoil. All topsoil shall be imported
from a commercial source for topsoil and must meet specifications for
imported topsoil.

14. The Contractor shall sawcut, remove and dispose of existing pavements,
curb and gutter, inlets, driveways and sidewalks offsite.

15. The Contractor shall install topsoil, place block sod, or seeding and
fertilize all areas disturbed by construction. The Contractor shall provide
whatever measures are needed, including temporary irrigation and
mowing, to ensure establishment of grass. Unless otherwise noted,
private lawn areas and parkways in front of private lawn areas disturbed
by construction shall be replaced with block sod of a similar grass to that
existing. All areas where sod is placed shall receive four (4) inches of
topsoil. All grass areas shall be fertilized at the rate of 400 lbs. per acre.

16. The Contractor shall be responsible for taking measures to minimize
damage to tree limbs, tree trunks, and tree roots along the route of the
project. All such measures shall be considered incidental work included in
the Contract Unit Price bid for applicable pipe or structure installation.
Contractor shall inspect each work site in advance and arrange to have
any tree limbs pruned that might be damaged by equipment operations.
The Engineer shall be notified at least 24 hours prior to any tree trimming
work. Nothing shall be stored over the tree root system within the drip line
limits of any tree. The Contractor shall employ a qualified landscaper for
all work required for tree care to ensure utilization of the best agricultural
practices and procedures.

17. A Stormwater Pollution Prevention Plan shall be implemented and


maintained during construction and shall be delivered to the engineer at
pre-construction conference.

18. All fences to remain unless otherwise noted.

19. Contractor shall field locate pipelines at connection points prior to ordering
or fabricating proposed pipe. Contractor shall verify pipe size, type,
elevation and horizontal location. Contractor shall make all necessary
adjustments to connect to existing line after approval from the engineer.

GENERAL NOTES FOR PAVING

1. All dimensions are to back of curb unless otherwise noted.

2. Embankments shall be constructed in accordance with NCTCOG


Standard Specification Item 203.7. Fill for embankments shall be
compacted in loose lifts of eight (8) inches thick maximum to at least 95%
of the maximum standard proctor density (ASTM D698) at minus two to
plus two percent of optimum moisture.

3. All street excavations shall be unclassified in accordance with NCTCOG


Item 203.4. All excavation shall be to the lines and grades shown on the
plans. The street excavation shall be made to the subgrade of the
roadway.

4. Concrete pavement shall be removed to existing joints.

GENERAL NOTES FOR STORM DRAINS

1. All storm drainage pipe and fittings shall be ASTM C-76, Class III
reinforced concrete pipe, unless otherwise noted on the plans.

2. Included in the unit price bid for reinforced concrete pipe and box shall be
trench excavation, embedment, backfill, fittings, wyes, tees, plugs, field
connections, pipe collars and temporary pavement repair. Dewatering as
required.

3. Pipe collars shall be installed at all pipe size and grade changes.

4. During drainage system construction, all inlets, pipes, and culverts shall
be protected from silt and soil deposits by use of silt fences at the
upstream end until construction is complete. Inlet protection must be of a
type that permits storm discharge to flow over the inlet protection and into
the inlet to prevent the possibility of clogging.

5. Prior to final acceptance contractor shall remove all silt, dirt, debris, etc.
from inlets, storm drains, culverts, manholes, and erosion control.

GENERAL NOTES FOR WATER LINES

1. Water Lines of twelve (12) inches (nominal) or less in diameter shall be


AWWA Standard C900, Class 150 (D.R. 18)

2. All water mains shall have a 48" minimum cover from finished grade to top
of pipe.

3. All water main pipe fittings will be in accordance with the City of Lewisville
Standard Details and specifications.

4. Fittings shall be either gray or ductile cast iron and shall be cement lined
inside and bituminous coated on the outside. Where connections are
made to pipes of dissimilar materials, suitable adaptors shall be used.

5. All ductile iron fittings shall be of mechanical joint type or slip joint type
and shall be Class D or Class 250 on sizes 12" or smaller in accordance
with AWWA Specification C-110-64 and Specification C-111-64.

6. All fire hydrants shall meet the City of Lewisville Standard Fire Hydrant
Specifications. Fire hydrants shall be Mueller Super Centurion, Clow
Medallion, Waterous WB-67, or equal with City approval.

7. Pipe joints shall be of the "push-on" type unless otherwise noted.

8. Valves 12-inch and smaller shall be gate valves. All valves shall be
flanged. Gate valves on laterals shall be attached to a flanged outlet, tee
or cross. Valves shall conform to AWWA C-509.

9. Valves, vault lids, fire hydrants and meter boxes shall be located outside
of sidewalk areas.

10. All water services shall be constructed per City of Lewisville Standard
Details and shall include a new water meter box.

11. All water services shall be 3/4" type "K" soft copper unless otherwise
specified in the plans. All connections to the main shall be 90 0 to the
vertical.

12. All trenching, embedment, and backfill will be incidental to the unit cost of
pipe.

13. When the horizontal distance between the water line joint and wastewater
main or lateral is less than nine feet, the wastewater pipe and joints shall
be constructed with pipe material having a minimum pressure rating of
150 psi.

14. A 2-inch "W" shall be chiseled or installed by other methods approved by


the City on the face of the curb to identify the location of the water service.

15. Ductile iron fittings for water line shall either be compact fittings (C153) or
standard (C110) as specified by NCTCOG.

TREE PROTECTION NOTES:

1. Unless shown on the plans to be removed, all trees within the construction
area shall be protected during construction with temporary fencing or other
measures as needed including pruning of limbs and roots. No pruning or
tree removal permitted without prior city approval for each occurrence.

2. Protective measures shall be taken prior to the start of any work in the
area and shall be maintained throughout the duration of the project.

3. Pruning of limbs to provide clearance for construction equipment shall


take place before construction begins to prevent damage to trees by
ripping and tearing of branches.

4. It is anticipated that in some instances trenches will encroach in the drip


line of protected trees. Encroachment in these instances is permitted.
Fencing will be installed outside the trench in such a manner as to limit the
amount of encroachment while allowing all necessary trench safety
measures.

5. Any roots exposed during construction activities shall be pruned flush with
the soil. If exposed root areas are not backfilled with topsoil within two
days of initial exposure, they shall be covered with organic material in a
manner that reduces the soil temperature and minimizes water loss due to
evaporation.

6. No topsoil dressing greater than 4 inches shall be permitted within drip line
areas.

7. Unless otherwise noted, payment for tree protection shall be subsidiary to


other items.
